Holiday cancellation form/certificate- £50
Certificate without medical exam- £15
Solicitors report- No charge under GDPR
Employment medical without exam- £50
Employment medical with exam- £90
Fitness to travel certificate- £25
Copy of patients GP notes- First set no charge, Further notes- £50
Insurance report- £150
